A documented immunization record must be included in the enrollment packet at your child's Reedley preschool or childcare provider. All additional forms, I.D. and Emergency Information, Physician’s Report, Health History, Medical Release, Statement of Parent’s Rights, must be completed and turned into your Reedley childcare provider or Reedley preschool along with signed Admission Agreement on or before your child's first day of attendance. Payment/Cancellation Terms
When a provider signs up for a premium listing on ChildCareSelect.com, they will be charged $9.95 per month on their credit/debit card, starting on the day they sign up. For example, if a provider signs up on January 4th, they will be billed $9.95 on January 4th (the day they signed up), and each subsequent month on the 4th day of those months to the credit/debit card that they originally signed up with.

Because the shortest month of the year contains 28 days, the 28th will be the last day of any month that we will bill. For example, if a provider signs up on January 30th, they will be charged $9.95 on January 30th (the day they signed up), and each subsequent month on the 28th day of those months.

A provider can cancel their membership at any time. Their credit/debit card will not be charged any more in the future, yet there will be no refunds given to prorate the month which they were last charged for.
