Immunizations Required for Rancho Mirage Childcare Providers and Rancho Mirage Preschools
Required by The State of California
California law requires all Children entering a childcare or preschool program in Rancho Mirage to have the following immunization: polio, DPT, MMR, HIB, HEP B and Varicella. A documented immunization record must be included in the enrollment packet at your child's Rancho Mirage preschool or childcare provider. All additional forms, I.D. and Emergency Information, Physician’s Report, Health History, Medical Release, Statement of Parent’s Rights, must be completed and turned into your Rancho Mirage childcare provider or Rancho Mirage preschool along with signed Admission Agreement on or before your child's first day of attendance. Any additional immunizations required at a later date are the responsibility of the parent or guardian. Please provide proof of immunizations to your child's Rancho Mirage preschool or Rancho Mirage child care provider.
