Immunizations Required for Buellton Childcare Providers and Buellton Preschools
Required by The State of California
California law requires all Children entering a childcare or preschool program in Buellton to have the following immunization: polio, DPT, MMR, HIB, HEP B and Varicella. A documented immunization record must be included in the enrollment packet at your child's Buellton preschool or childcare provider. All additional forms, I.D. and Emergency Information, Physician’s Report, Health History, Medical Release, Statement of Parent’s Rights, must be completed and turned into your Buellton childcare provider or Buellton preschool along with signed Admission Agreement on or before your child's first day of attendance. Any additional immunizations required at a later date are the responsibility of the parent or guardian. Please provide proof of immunizations to your child's Buellton preschool or Buellton child care provider.
